Z-Track Technique for Intramuscular Injections

Clinical Benefits and Evidence

  • The American Academy of Medical Specialists recommends using the Z-track technique for deep intramuscular injections in areas with adequate muscle mass, as it is designed to prevent medication leakage and reduce tissue damage, and should not be used for subcutaneous injections where tissue displacement could cause intramuscular injection 1, 2
  • The Z-track technique is not recommended for subcutaneous injections, such as insulin, where intramuscular injection must be avoided, according to the Mayo Clinic Proceedings 2

Common Pitfalls and How to Avoid Them

  • The Z-track technique should not be confused with the skin traction and pressure technique, which involves deep pressure to the muscle but not lateral displacement, as stated in the Mayo Clinic Proceedings 1, 2
