Management of COPD Exacerbations
Antibiotic Selection
- The European Respiratory Society recommends amoxicillin or amoxicillin/clavulanate as the first-line antibiotic for COPD exacerbations in patients without penicillin allergy, due to its coverage of common pathogens such as Haemophilus influenzae, Streptococcus pneumoniae, and Moraxella catarrhalis 1, 2, 3, 4, 5
- Amoxicillin/clavulanate is preferred due to its coverage of beta-lactamase producing organisms, which is particularly important given local resistance patterns 1, 5
- The recommended dosing for amoxicillin/clavulanate is 1000 mg every 8 hours orally for 7 days 3, 4
Corticosteroid Management
- The European Respiratory Society recommends prednisone 30-40 mg orally daily for 10-14 days for COPD exacerbations, as it accelerates recovery of lung function and reduces treatment failure rates 1, 2
Monitoring and Follow-Up
- Clinical response to treatment should be assessed within 2-3 days, with expectations of improvement in symptoms such as dyspnea, sputum production, and oxygenation 2, 3, 6
- If no improvement is seen by day 3, treatment failure should be considered and alternative diagnoses or resistant organisms should be investigated 3, 6
