Management of Paradoxical Insomnia
Medication Optimization
- The American Academy of Sleep Medicine does not recommend olanzapine for primary insomnia due to weak evidence and concerning adverse effect profile, and instead suggests optimizing other medications such as mirtazapine 1, 2
- Increasing mirtazapine to 30mg at bedtime provides potent sleep promotion, improves sleep architecture, and addresses both anxiety and depressive symptoms that may underlie insomnia, with optimal effects seen at this dose 1
Non-Pharmacological Interventions
- Cognitive Behavioral Therapy for Insomnia (CBT-I) must be started alongside any medication adjustment, as it represents the standard of care with superior long-term outcomes compared to pharmacotherapy alone, and includes components such as stimulus control therapy, sleep restriction therapy, sleep hygiene optimization, and cognitive restructuring 2, 3
Hypnotic Agents
- For combined sleep onset and maintenance insomnia, eszopiclone 2-3mg or zolpidem 10mg may be added as first-line agents, with consideration of patient factors such as age and substance use history 2, 3
- For predominantly sleep-onset insomnia, zaleplon 10mg or ramelteon 8mg may be considered, while for predominantly sleep-maintenance insomnia, low-dose doxepin 3-6mg may be effective 2, 3
Safety Considerations
- The American Academy of Sleep Medicine recommends avoiding the combination of olanzapine and mirtazapine without clear psychiatric indication beyond insomnia, due to significant weight gain and metabolic disturbance 1, 2
- Over-the-counter antihistamines should not be used due to lack of efficacy data and anticholinergic burden, and benzodiazepines should not be added as first-line agents due to dependence risk and cognitive impairment 2, 3
Monitoring and Reassessment
- Sleep response should be assessed after 1 week of mirtazapine dose increase, and weight, fasting glucose, and lipid panel should be monitored at baseline and 3 months if continuing olanzapine 2, 3
- The need for medication should be reassessed after 3-4 months, with goal of tapering hypnotics if insomnia improves, while maintaining CBT-I techniques 2, 3
