Gabapentin Dosing and Administration

Introduction to Gabapentin

  • The American Academy of Neurology recommends initiating gabapentin at lower doses (300 mg) and gradually titrating to minimize side effects 1
  • When taking gabapentin for neuropathic pain, the maximum single dose is generally 1200 mg, as part of the maximum daily dose of 3600 mg divided into three doses 1, 2

Dosing Considerations

  • Initial dosing should start at 100-300 mg at bedtime or 100-300 mg three times daily, with increases by 100-300 mg every 1-7 days as tolerated 1
  • For neuropathic pain, treatment should be initiated at low dosages with gradual titration until pain relief, dose-limiting adverse effects, or the maximum dose of 3600 mg/day in 3 divided doses is reached 2
  • An adequate trial of gabapentin for pain can require 2 months or more to determine full efficacy 1

Special Considerations

  • Side effects can be reduced by starting with lower dosages and titrating cautiously 2
  • For unexplained chronic cough, a therapeutic trial of gabapentin can be considered with dose escalation beginning at 300 mg once daily up to a maximum tolerable daily dose of 1800 mg in two divided doses 3

Maximum Dose of Gabapentin

Dosing Guidelines for Gabapentin

  • The American Geriatrics Society recommends that for neuropathic pain, the effective dose range of gabapentin is typically between 1800-3600 mg/day 4, 5
  • In controlled studies, the effective gabapentin dose for pain is usually between 900 mg/day and 3600 mg/day in two to three divided doses 4

Dose Titration

  • The National Comprehensive Cancer Network recommends that treatment should be initiated with a low starting dose and gradually titrated to minimize side effects, with dose titration continuing until benefit occurs or side effects begin to appear 5, 4, 6

Special Populations

Elderly Patients

  • The American Geriatrics Society suggests that lower doses may be more appropriate in older adults due to potential for decreased renal function, and the effective dose in older adults may be lower than the standard ranges due to increased sensitivity to side effects 4
