Treatment of Intertrigo
Core Treatment Strategy
- The American College of Physicians recommends keeping affected skin folds dry as the cornerstone of treatment for intertrigo, especially in overweight or diabetic patients, combined with topical antifungal therapy when secondary candidal infection is present 1, 2
- The treatment guidelines suggest that all three antifungal agents (clotrimazole, miconazole, and nystatin) are equally effective for candidal intertrigo, and the choice should be based on availability and cost 2
- Clotrimazole 1% cream, miconazole 2% cream, and nystatin cream are recommended topical antifungal therapies, applied twice daily for 7-14 days 1, 2
- Topical therapy alone may be insufficient in moderate-to-severe infections or immunocompromised patients, and oral fluconazole 100-200 mg daily should be considered in these cases 2
Management of Diabetes
- The American Diabetes Association recommends targeting 3-7% weight loss to improve glycemia, blood pressure, and reduce medication needs 3, 4, 5
- Structured behavioral interventions with at least 16 counseling sessions over 6 months, focusing on a 500-750 kcal/day energy deficit, are recommended for weight management 3, 6
- Anthropometric measurements should be monitored at least every 3 months during active weight management 4, 5
- Continuing weight management efforts with monthly contact and ongoing support is crucial for sustained benefit 3, 6
