Semaglutide Safety and Efficacy

Introduction to Semaglutide

  • The American College of Cardiology notes that the FDA and European Medicines Agency have not identified a causal link between GLP-1 receptor agonists and either pancreatitis or pancreatic cancer 1

Gastrointestinal Effects

  • The American College of Cardiology recommends that nausea, vomiting, and diarrhea, the most frequently reported side effects of semaglutide, can be minimized by starting at the lowest dose and up-titrating gradually 2, 1
  • The American College of Cardiology warns of an increased risk of gallbladder disease, including cholelithiasis and acute cholecystitis, associated with semaglutide use 3, 4

Diabetic Retinopathy Complications

  • The American College of Cardiology suggests that patients with pre-existing proliferative retinopathy are at higher risk of diabetic retinopathy complications when using semaglutide, and recommends eye examinations before starting therapy 2, 3

Cardiovascular and Metabolic Effects

  • The American College of Cardiology notes that semaglutide can cause modest elevations in heart rate, and typically decreases blood pressure by 1-6 mm Hg 3, 1, 4
  • The American Heart Association reports that semaglutide reduced major cardiovascular events in adults with obesity/overweight and established cardiovascular disease in the SELECT trial 6

Renal Considerations

  • The American College of Cardiology advises to use caution in patients with severe renal impairment or end-stage renal disease when initiating or increasing the dose of semaglutide due to potential risk of acute kidney injury 1, 4

Hypoglycemia Risk

  • The American College of Cardiology warns that semaglutide is unlikely to cause hypoglycemia on its own, but may do so when combined with insulin or insulin secretagogues, and recommends lower doses of these concomitant medications 1, 7

Contraindications and Drug Interactions

  • The American College of Cardiology lists absolute contraindications for semaglutide, including personal or family history of medullary thyroid carcinoma and Multiple Endocrine Neoplasia syndrome type 2 (MEN 2) 7
  • The American College of Cardiology advises against coadministering semaglutide with DPP-4 inhibitors, and to use caution in patients with clinically significant gastroparesis or prior gastric surgery 2, 7

Monitoring Recommendations

  • The American Diabetes Association recommends assessing efficacy and safety at least monthly for the first 3 months, then at least quarterly, and monitoring for signs/symptoms of pancreatitis, gallbladder disease, and thyroid tumors 4, 8, 5
