Maximum Dosage of Glipizide

Dosing Guidelines

  • The maximum approved daily dose of glipizide is 40 mg for immediate-release (IR) formulation and 20 mg for extended-release (XL) formulation, as recommended by the American Diabetes Association 1
  • The maximum daily dose for extended-release (XL) formulation is 20 mg, as per the American Diabetes Association guidelines 1

Special Populations and Considerations

  • For patients with chronic kidney disease (CKD) and eGFR > 50 mL/min/1.73 m², no dose adjustment is needed, according to the National Kidney Foundation 2, 3
  • For patients with CKD and eGFR < 50 mL/min/1.73 m², initiation should be conservative (e.g., 2.5 mg once daily) and titration should be slow to avoid hypoglycemia, as recommended by the National Kidney Foundation 3
  • Glipizide can be used in patients with renal impairment with appropriate dose adjustments, whereas glyburide should be avoided, as suggested by the National Kidney Foundation 3

Monitoring and Safety

  • Hypoglycemia is the most common adverse effect, particularly in elderly patients or those with renal impairment, as reported by the National Kidney Foundation 3

Cost Considerations

  • The median monthly cost for maximum approved daily dose of glipizide 10 mg (IR) was $75 (AWP) and $5 (NADAC) in 2020, according to the American Diabetes Association 1
  • The median monthly cost for glipizide 10 mg (XL) was $48 (AWP) and $15 (NADAC) in 2020, as reported by the American Diabetes Association 1
