Hypertension Management Principles

Standard Approach to Hypertension Management

  • The American College of Cardiology recommends first-line agents including thiazide diuretics, ACE inhibitors, ARBs, and calcium channel blockers for hypertension management 1
  • The European Heart Journal suggests that combination therapy is often needed, with upfront low-dose combinations recommended for hypertension 2
  • Single-pill combinations are preferred when multiple agents are needed to improve adherence, as recommended by the European Heart Journal 2
  • Beta-blockers are not recommended as first-line agents unless there are specific indications, according to the American College of Cardiology 1
  • For resistant hypertension, spironolactone is often added as a fourth agent, as suggested by the European Heart Journal and the American College of Cardiology 1, 2

Considerations for Specific Agents

  • Direct vasodilators like hydralazine should generally be considered only as fourth-line or later additions due to adverse effects like fluid retention and tachycardia, as noted in Drugs 4, 5
