Drug Interaction Between Risperidone and Montelukast

Metabolic Pathways and Interaction Potential

  • Risperidone is primarily metabolized by cytochrome P450 2D6, with some contribution from CYP3A4 1

Clinical Safety Considerations

  • The European Position Paper on Rhinosinusitis notes that various neuropsychiatric events have been reported with anti-leukotrienes, though evidence of causation remains conflicting 2, 3

Monitoring and Clinical Context

  • Extrapyramidal symptoms from risperidone, especially at doses >6 mg/24h, should be monitored 1
  • Patients with psychiatric conditions requiring risperidone (schizophrenia, bipolar disorder, behavioral disturbances) may be treated with this combination 1
  • The American College of Physicians, as reported in Annals of Internal Medicine, suggests that montelukast may be used for concurrent asthma or allergic rhinitis, and this combination may be safe with appropriate monitoring 4

Important Caveats

  • No dose adjustments are required for either medication when used together 1
  • Alternative asthma therapies (intranasal corticosteroids for rhinitis, inhaled corticosteroids for asthma) may be considered if neuropsychiatric symptoms emerge, as these are more effective first-line options anyway 4
