Management of Severe Fulminant Ulcerative Colitis with Toxic Megacolon

Introduction to Surgical Management

  • The American College of Surgeons recommends surgery as the definitive treatment for patients with toxic megacolon who have failed medical treatment, with subtotal colectomy with ileostomy being the procedure of choice 1, 2, 3, 4, 6

Indications for Surgery

  • Patients with toxic megacolon showing no clinical improvement after 24-48 hours of medical treatment require mandatory surgery, as delaying surgery significantly increases the risk of perforation and associated high mortality 1, 2, 4, 5
  • Toxic megacolon with failure of medical treatment is an absolute indication for emergency surgery, according to the World Journal of Emergency Surgery 1, 2, 3, 4, 7

Surgical Procedure

  • Subtotal colectomy with ileostomy is the surgical treatment of choice for acute severe ulcerative colitis patients who are non-responders to medical treatment, involving removal of the colon and creation of an end ileostomy 1, 2, 3, 4, 6

Timing of Surgery

  • Surgery should not be delayed beyond 48-72 hours from diagnosis of toxic megacolon if there is no clinical improvement, as prolonged observation increases the risk of toxic megacolon progression and perforation with high mortality rates 1, 4

Avoiding Common Pitfalls

  • The most critical error is delaying surgery while attempting additional medical therapy in a patient who has already failed medical treatment and has toxic megacolon, which substantially increases mortality risk 1, 4, 5, 6
