Treatment of Postpartum Acne

Topical Therapy

  • The American Academy of Dermatology recommends topical salicylic acid (0.5-2%) as a safe and effective comedolytic agent for postpartum acne treatment 1
  • Fixed-combination products, such as erythromycin 3%/BP 5% or clindamycin 1%/BP 5%, enhance compliance and are considered safe during lactation, with a pregnancy category B for clindamycin 2, 1
  • Topical dapsone 5% gel is effective for inflammatory acne in adult females and is safe during lactation 1

Treatment Algorithm

  • The American Academy of Dermatology suggests that topical antibiotics, such as clindamycin or erythromycin, should never be used as monotherapy due to the risk of resistance, and should always be paired with benzoyl peroxide 3
  • Maintenance therapy with benzoyl peroxide or azelaic acid is crucial to prevent recurrence of postpartum acne 3

Safety Considerations

  • The risk of pseudomembranous colitis with topical clindamycin is extremely low, but has been reported with long-term widespread use, according to the American Academy of Dermatology 2
