Metoprolol for Ventricular Tachycardia in Patients with Asthma

Treatment Approach for VT in Asthmatic Patients

  • Amiodarone is the preferred agent for hemodynamically stable VT in patients with asthma, as it does not have significant bronchospastic effects, according to the American Heart Association 1
  • Procainamide can be considered for hemodynamically stable monomorphic VT at 20-50 mg/min until arrhythmia is suppressed, as recommended by the American College of Cardiology 2

Administration Guidelines if Metoprolol Must Be Used

  • The American Heart Association recommends starting with the lowest possible dose of metoprolol (e.g., 5 mg IV over 1-2 minutes) 1, 2

Evidence on Safety

  • Small studies have shown that metoprolol can be used in patients with serious pulmonary disease after correction of hypoxia or other signs of acute decompensation, with a moderate level of evidence 3

Special Considerations

  • Metoprolol is contraindicated in decompensated heart failure, according to the American College of Cardiology 4
  • The American Heart Association recommends avoiding metoprolol in patients with severe conduction abnormalities or sinus node dysfunction 3
