Management of Hypertension in Grade 2 Hypertension
Classification and Initial Management
Patients with blood pressure 165/80 mmHg should immediately start pharmacological therapy along with lifestyle modifications as this falls under grade 2 hypertension (≥160/100 mmHg). 1
In grade 2 hypertension, medication therapy should be initiated immediately alongside lifestyle modifications. 1
Blood pressure of 165/80 mmHg is classified as grade 2 hypertension (≥160/100 mmHg) requiring immediate intervention. 1, 2
Lifestyle Modifications
The European Society of Cardiology recommends weight reduction to achieve BMI 20-25 kg/m² and waist circumference <94 cm for hypertension management. 3
The European Society of Cardiology recommends limiting alcohol consumption (<100 g/week) or avoiding it completely as part of hypertension management. 3
The European Society of Cardiology recommends smoking cessation as an important lifestyle modification for hypertension management. 3
The European Society of Cardiology recommends limiting free sugar consumption to a maximum of 10% of total energy intake as part of hypertension management. 3
Regular physical activity consisting of aerobic exercise for 30 minutes daily, 5-7 days per week is recommended for hypertension management. 1
Pharmacological Therapy
For grade 2 hypertension (≥160/100 mmHg), guidelines recommend initiating therapy with a combination of 2 medications. 1, 4
The recommended combination therapy includes an ACE inhibitor or ARB plus either a thiazide diuretic or dihydropyridine CCB. 1, 3
For non-Black patients, initial therapy should begin with a combination of ARB/ACE inhibitor plus dihydropyridine CCB. 1
If blood pressure remains uncontrolled with 3 medications, consider adding spironolactone as a fourth agent. 6
Blood Pressure Targets
The European Society of Cardiology recommends a target blood pressure of <130/80 mmHg for optimal management. 3, 4
At minimum, blood pressure should be reduced to <140/90 mmHg according to guidelines. 7
Guidelines recommend achieving a minimum blood pressure reduction of 20/10 mmHg from baseline values. 1
The target blood pressure should be achieved within 3 months of initiating therapy. 1
Monitoring and Follow-up
Blood pressure response to treatment should be evaluated 2-4 weeks after medication changes. 5
Home blood pressure monitoring should be implemented to ensure consistent blood pressure control. 1
Follow-up visits should be scheduled every 3-6 months after blood pressure is controlled. 7
Important Considerations
The European Society of Cardiology advises against combining two renin-angiotensin system inhibitors (such as ACE inhibitor and ARB) in hypertension management. 3
The European Society of Cardiology recommends considering fixed-dose combination pills to improve medication adherence in hypertension management. 3
