Thiazide Diuretic Therapy
Single-Agent Thiazide Diuretics
- Chlorthalidone is increasingly recognized as a preferred thiazide-like diuretic due to its use in major cardiovascular outcome trials 1
- Indapamide is recommended alongside chlorthalidone as a preferred diuretic due to its longer duration of action 1
Combination Products with Thiazide Diuretics
- The American Heart Association recommends the use of combination products with thiazide diuretics, such as ACE inhibitor + hydrochlorothiazide combinations, including Lotensin HCT, Capozide, and Vaseretic 2
- The American Heart Association also recommends the use of ARB + hydrochlorothiazide combinations, including Atacand HCT, Teveten-HCT, and Avalide 2
Important Clinical Considerations
- The equivalent dose of hydrochlorothiazide for 25 mg chlorthalidone is 50 mg, as chlorthalidone is approximately twice as potent 1
- Successful morbidity trials used doses equivalent to 25-50 mg of hydrochlorothiazide or 12.5-25 mg of chlorthalidone 2
- The use of generic thiazide diuretics, including hydrochlorothiazide and chlorthalidone, can significantly reduce prescription costs 2
- Separate generic prescriptions may cost less than non-generic fixed-dose combinations 3
