Lamotrigine Titration Guidelines

Initial Dosing and Titration Schedule

  • The American Academy of Neurology recommends initial dosing of Lamotrigine to start at 25 mg daily, with a slow titration schedule to minimize the risk of serious rash 1
  • After one month (4 weeks) at 25 mg daily, the dose can be increased to 50 mg daily, as recommended by the Mayo Clinic 1
  • Further titration should continue gradually, with increases of 25-50 mg every 1-2 weeks as tolerated, according to the Mayo Clinic guidelines 1

Administration Considerations

  • Lamotrigine is available in regular tablets that should be swallowed whole with water, and dissolving tablets are also available for patients who have difficulty swallowing pills 2
  • Both formulations are contraindicated in patients with previous lamotrigine-induced rash, as stated by Praxis Medical Insights 2
