Sertraline Dosing Guidelines for Pediatric Anxiety

Initial Dosing and Titration

  • The American Academy of Child and Adolescent Psychiatry recommends starting sertraline at 25 mg once daily as a test dose, then increasing to 50 mg daily after one week if well-tolerated, with subsequent dose adjustments every 1-2 weeks up to a maximum of 200 mg/day based on response and tolerability 1
  • Dose increases should be made at 1-2 week intervals due to sertraline's 24-hour elimination half-life, with increases in 25 mg increments to avoid exceeding the optimal dose and minimize adverse effects 1, 2

Expected Response and Monitoring

  • Statistically significant improvement may occur within 2 weeks, though this may not be clinically meaningful, and clinically significant improvement typically appears by week 6 1, 2
  • Maximal improvement occurs by week 12 or later, supporting the rationale for slow dose titration, and an adequate trial requires 8 weeks at an optimal dose before determining non-response 1, 2
  • Close monitoring is essential in the first months of treatment and following each dosage adjustment, with weekly monitoring during dose adjustments recommended 1, 2

Safety Considerations

  • All SSRIs carry a boxed warning for suicidal thinking and behavior through age 24 years, and the pooled absolute rate for suicidal ideation is 1% for antidepressants versus 0.2% for placebo 1, 2
  • Behavioral activation is more common in younger children than adolescents and in anxiety disorders compared to depression, and may occur early in treatment, with dose increases, or when doses are escalated too rapidly 1, 2
  • Most adverse effects emerge within the first few weeks and include nausea, diarrhea, headache, insomnia, dizziness, and fatigue, with serious but rare effects including hypomania/mania, seizures, abnormal bleeding, and serotonin syndrome 1, 2

Combination Treatment and Administration

  • Combination treatment with CBT plus sertraline is superior to either treatment alone for pediatric anxiety disorders, with 80.7% of children responding to combination therapy versus 54.9% to sertraline alone and 23.7% to placebo 3, 4
  • Administer sertraline once daily, either morning or evening, with parental oversight of medication administration paramount in this age group 1, 2, 3

Discontinuation Protocol

  • Taper sertraline gradually over a minimum of 2-4 weeks for short-term therapy, and for long-term therapy, taper over several months, with reinstitution of the previous dose and slowing of the taper rate if moderate to severe withdrawal symptoms occur 2
