Treatment of ADHD with Comorbid Depression and Anxiety

Primary Treatment Algorithm

  • The American Academy of Child and Adolescent Psychiatry confirms that stimulants are effective for treating ADHD with comorbid anxiety, with better treatment responses to stimulants than those without anxiety, in patients with ADHD 2, 1
  • Stimulants offer rapid onset of action, allowing quick assessment of whether ADHD symptoms have remitted, and reduction in ADHD-related morbidity often produces substantial improvement in both depressive and anxiety symptoms without additional treatment, in patients with ADHD and comorbid depression and anxiety 2, 1
  • The American Academy of Child and Adolescent Psychiatry recommends starting with extended-release stimulant formulations to achieve all-day symptom control and minimize rebound effects, in patients with ADHD and comorbid depression and anxiety 1

Alternative First-Line Option

  • Atomoxetine is an alternative first-line option for patients with severe anxiety, active substance use disorder, pre-existing sleep disorders, or stimulant non-response or intolerance, and provides "around-the-clock" effects without rebound, which may benefit patients with mood instability 1, 4

Important Safety Considerations

  • The American Academy of Child and Adolescent Psychiatry recommends exercising caution when combining SSRIs with stimulants due to serotonin syndrome risk, and starting SSRIs at low doses and increasing slowly, monitoring especially in first 24-48 hours after dosage changes, in patients with ADHD and comorbid depression and anxiety 3, 1
  • Escitalopram and citalopram have the least CYP450 enzyme effects and lowest drug interaction propensity, according to the American Academy of Child and Adolescent Psychiatry 1, 3

Adjunctive Options for Insufficient Response

  • The American Academy of Child and Adolescent Psychiatry recommends considering alpha-2 agonists as adjunctive treatment for insufficient response to stimulants or atomoxetine, which addresses both ADHD symptoms and emotional dysregulation, in patients with ADHD and comorbid depression and anxiety 1, 4
