Treatment of OCD in the Context of Bipolar Disorder

Primary Treatment Approach

  • The American Academy of Child and Adolescent Psychiatry recommends traditional mood stabilizers (lithium, valproate) and/or atypical antipsychotics as the foundation of treatment for the bipolar component 1, 2
  • Cognitive-behavioral therapy (CBT) with exposure and response prevention (ERP) is the psychological treatment of choice for the OCD component, with 10-20 sessions typically recommended 3, 4
  • Patient adherence to between-session homework (ERP exercises) is the strongest predictor of good outcomes 4

Pharmacological Management

  • Mood stabilizers (lithium, valproate) should be established first to control bipolar symptoms, with aripiprazole augmentation showing particular promise for treating comorbid OCD-bipolar disorder 1
  • If OCD symptoms remain severe after mood stabilization, carefully consider adding an SSRI, starting with lower doses and increasing gradually 5

Treatment Algorithm

  • Evaluate severity of both bipolar disorder and OCD symptoms, and identify current mood state (manic, depressive, mixed, or euthymic) 6, 1
  • Begin with mood stabilizers (lithium, valproate) and/or atypical antipsychotics, and achieve stable mood before aggressively targeting OCD symptoms 2

Long-term Management

  • Most patients with bipolar disorder require ongoing medication therapy to prevent relapse, with maintenance therapy often needed for 12-24 months 7
  • Regular reassessment of the treatment regimen is essential to balance symptom control with side effect management 1, 3

Treatment of Comorbid Bipolar Disorder and OCD

Initial Treatment Strategy

  • Cognitive-behavioral therapy (CBT) with exposure and response prevention (ERP) is the psychological treatment of choice for OCD, with patient adherence to between-session ERP homework exercises being the strongest predictor of good outcomes, according to Nature Reviews Disease Primers 8

Special Considerations and Pitfalls

  • For treatment-resistant OCD symptoms, consider glutamatergic medications as augmentation agents, such as N-acetylcysteine, which has the largest evidence base, and memantine, as suggested by Nature Reviews Disease Primers 9
  • Neuromodulation approaches, including FDA-approved deep repetitive transcranial magnetic stimulation (rTMS) and deep brain stimulation (DBS), may be considered for severe, treatment-resistant OCD, as recommended by Nature Reviews Disease Primers 9 and 10

Long-term Management

  • Evaluate quality of life regularly, as both OCD and bipolar disorder significantly reduce quality of life across multiple domains, and address comorbid depression aggressively when present, as depressive symptoms mediate the relationship between OCD and impaired quality of life, according to Nature Reviews Disease Primers 11

Treatment Monitoring

  • Consider family involvement in treatment when possible to address family accommodation to OCD symptoms, as suggested by Nature Reviews Disease Primers 8 and 12
