Alternatives to Truvada for HIV Prevention

Alternative Regimens for HIV Prevention

  • The American College of Cardiology is not applicable here, however, the CDC recommends Descovy (emtricitabine/tenofovir alafenamide) as the most appropriate alternative for patients experiencing hives from Truvada, as it maintains similar efficacy while potentially reducing adverse reactions 1
  • For post-exposure prophylaxis (PEP), Descovy can be combined with an integrase strand transfer inhibitor such as bictegravir or dolutegravir as part of a complete regimen, as recommended by the CDC 2
  • The standard dosing for Descovy is one tablet (200 mg emtricitabine/25 mg tenofovir alafenamide) taken orally once daily with or without food, according to the Praxis Medical Insights 1
  • Bictegravir/emtricitabine/tenofovir alafenamide (BIC/FTC/TAF) as a single-tablet regimen is another preferred option for adults requiring HIV prevention, as per the MMWR Recommendations and Reports 2
  • For patients who cannot tolerate either tenofovir formulation, regimens containing dolutegravir plus lamivudine may be considered, based on recommendations from the MMWR and Clinical Gastroenterology and Hepatology 2, 3

Considerations for Special Populations

  • For patients with HIV/HBV co-infection who cannot tolerate Truvada, Descovy remains important as both emtricitabine and tenofovir have activity against hepatitis B virus, according to Clinical Gastroenterology and Hepatology and Hepatology 3, 4
  • If tenofovir cannot be used at all, entecavir might be considered as part of an alternative HIV regimen for managing HBV co-infection, as suggested by Clinical Gastroenterology and Hepatology 5
  • Lamivudine alone should be avoided as the only anti-HBV active agent due to high incidence of resistance, as noted by Clinical Gastroenterology and Hepatology 6

Monitoring After Switching Medications

  • HIV testing should be performed before initiating any alternative regimen, as recommended by Praxis Medical Insights 1
  • Follow-up HIV testing is recommended at 4-6 weeks and 12 weeks after exposure, according to Praxis Medical Insights 1
  • Monitor for any new adverse reactions after switching to the alternative medication, as advised by MMWR Recommendations and Reports 2

Clinical Pearls and Pitfalls

  • Do not discontinue PrEP or PEP without replacement - maintain continuous protection by immediately switching to an alternative regimen, as recommended by MMWR Recommendations and Reports 2
  • Consider drug interactions when selecting alternative regimens, particularly with medications that may affect renal function, according to Praxis Medical Insights 1

Alternative HIV Regimens for Patients with Truvada Allergy

Preferred Alternative Regimens

  • The American Academy of HIV Medicine recommends using Abacavir/lamivudine (ABC/3TC) as a replacement for the tenofovir backbone when combined with an integrase inhibitor, with the critical caveat that HLA-B*5701 testing is mandatory before prescribing abacavir 7
  • The guideline society suggests that Abacavir/lamivudine (ABC/3TC) can be used as a viable alternative regimen, with the strength of evidence supporting its use in treatment-naïve adults 7

Alternative Backbone Options

  • The HIV treatment guidelines recommend using Dolutegravir plus emtricitabine/tenofovir alafenamide (DTG + FTC/TAF) as an equally effective alternative regimen, with non-inferior efficacy to other regimens 7
