Benralizumab Treatment for Severe Asthma and Hypereosinophilic Syndrome

Efficacy and Application

  • Benralizumab has shown significant lowering of eosinophil counts in both serum and tissue for hypereosinophilic disorders, with 74% of patients having sustained response at 48 weeks and no adverse events limiting treatment 1, 2
  • The European Respiratory Society recommends therapeutic drug monitoring when using benralizumab 3
  • The American Gastroenterological Association does not currently recommend benralizumab as first-line therapy for eosinophilic esophagitis (EoE), suggesting its use only in the context of clinical trials 4

Safety and Monitoring

  • Common adverse events of benralizumab include worsening asthma, nasopharyngitis, and injection site reactions, with significant reduction in blood eosinophil counts typically seen within 24 hours and maintained throughout treatment 1, 2

Risk of Eosinophilic Pneumonia with Anti-IL5 Drugs

Safety Profile of Anti-IL5 Agents

  • No major safety signals have been identified in randomized controlled trials of mepolizumab for hypereosinophilic syndromes or severe eosinophilic asthma 5, 6, 7
  • Benralizumab demonstrated 74% sustained response at 48 weeks in hypereosinophilic disorders with no adverse events limiting treatment 8

Clinical Context and Efficacy Considerations

  • For eosinophilic esophagitis, current guidelines recommend anti-IL5 therapy only in clinical trials, not due to safety concerns but because of limited symptomatic improvement despite histologic response 5, 6, 8
  • The American Gastroenterological Association found that 94.4% of patients failed to achieve histologic remission with anti-IL5 therapy for eosinophilic esophagitis, though this reflects efficacy rather than safety issues 5, 7
