Antidepressant-Associated Weight Changes

Introduction to Weight Changes

  • Antidepressants have varying effects on weight, with fluoxetine and sertraline being the SSRIs least likely to cause weight gain, while bupropion is consistently associated with the least weight gain or even weight loss among all antidepressants 1
  • Not all weight-neutral options are suitable for all patients with depression, and individual responses to SSRIs can vary significantly 1
  • Bupropion may be activating and can exacerbate anxiety or be inappropriate for patients with bipolar disorder, despite its benefits for weight management 1

Medication-Specific Weight Effects

  • Fluoxetine is associated with weight loss during short-term use and weight neutrality with long-term use 1
  • Sertraline is associated with weight loss during short-term use and weight neutrality with long-term use 1
  • Bupropion consistently promotes weight loss by suppressing appetite and reducing food cravings, and is the only antidepressant shown to consistently decrease body weight 1
  • Paroxetine is associated with the greatest risk for weight gain within the SSRI class 1
  • Mirtazapine promotes appetite and weight gain 2

Treatment Recommendations

  • For patients with significant weight concerns, bupropion is recommended as a first choice due to its association with the least weight gain or even weight loss, if there are no contraindications like anxiety disorders or seizure risk 1
  • Fluoxetine or sertraline are recommended as second choices due to their weight-neutral effects with long-term use 1
  • Paroxetine, escitalopram, and mirtazapine should be avoided if weight gain is a significant concern 1, 2
  • The choice of agent must be guided by the specific type of depression, medication adherence, and individual patient factors, as not all weight-neutral options are appropriate for all patients 1
  • Estimated 6-month weight differences compared to sertraline are: 1
  • Sertraline has less effect on metabolism of other medications, according to the American family physician 2
