Therapeutic Applications of Ursodeoxycholic Acid (UDCA)

Primary Indications

  • The American Association for the Study of Liver Diseases recommends UDCA at 13-15 mg/kg/day as the established first-line treatment for primary biliary cirrhosis (PBC) based on multiple controlled trials 1, 2

Mechanism of Action

  • UDCA exerts anticholestatic effects through stimulation of impaired hepatocellular secretion via posttranscriptional mechanisms, and stimulation of ductular alkaline choleresis 1
  • UDCA changes bile acid composition from hydrophobic to more hydrophilic, reducing the amount of toxic bile acids 3

Clinical Effects in Primary Biliary Cirrhosis

  • UDCA significantly decreases serum bilirubin, alkaline phosphatase, gamma-glutamyl transferase, cholesterol, and immunoglobulin M levels in patients with PBC 1, 2
  • Long-term UDCA treatment delays histological progression of PBC when started at an early stage 1
  • UDCA treatment is associated with a significant reduction in the likelihood of liver transplantation or death in patients with moderate to severe PBC 1, 2

Other Potential Applications

  • UDCA may be considered for treatment of maternal pruritus in intrahepatic cholestasis of pregnancy (ICP) 4
  • In ICP with serum bile acid concentrations >40 μmol/L, UDCA may reduce the risk of spontaneous preterm birth and potentially protect against stillbirth 4

Dosing Considerations

  • For PBC: 13-15 mg/kg/day is the recommended dose, according to the European Association for the Study of the Liver 1, 2
  • For ICP: 10-15 mg/kg/day divided in 2-3 daily doses is recommended 2

Monitoring

  • Regular monitoring of liver biochemistry is essential to assess treatment response, as recommended by the American Association for the Study of Liver Diseases 2
  • In PBC, AMA-positive individuals with normal liver tests should undergo annual reassessment of biochemical markers of cholestasis 1
  • In ICP, serum bile acids should be checked at least weekly as they may continue to rise with advancing gestation 4
