Dosing of PEG-Asparaginase in Acute Lymphoblastic Leukemia in Older Adults
Dosing According to Clinical Evidence Guidelines
Standard FDA Dose vs. Adapted Doses for Older Adults
- The National Comprehensive Cancer Network recommends a dose of 2,500 UI/m² for PEG-asparaginase, although reduced doses of 2,000 UI/m² or less are suggested for older adults due to a significantly elevated toxicity profile in this age group 1, 2
- The American College of Clinical Oncology suggests that the USC/MSKCC trial, which used 2,000 UI/m² in adults aged 18-60, demonstrated that this dose was lower than the FDA-approved dose to reduce toxicity in adults 1, 2, 3, 4
- The reduction in dose was implemented to adjust for differences in enzymatic activity between adults and children 1, 2
Recommended Dosing Scheme
For Older Adults (≥40-50 years)
- The National Comprehensive Cancer Network recommends initiating with conservative doses of 1,000-2,000 UI/m², with intervals of ≥4 weeks between doses 1, 2
- The initial dose should be 1,000-2,000 UI/m² IV, with a minimum interval of ≥4 weeks between doses 1, 2
- The maximum number of doses should be up to 6 total doses scheduled over the course of treatment 1, 2
- Mandatory premedication with hydrocortisone for hypersensitivity prophylaxis, followed by 1-2 weeks of oral steroids, is recommended 1, 2
Tolerability Data in Clinical Practice
- In the USC/MSKCC trial using 2,000 UI/m², patients received an average of 3.8 doses per patient, with only 45% completing the 6 scheduled doses 1, 2
- 20% of patients discontinued treatment due to toxicity 1, 2
Contraindications and Warnings
- The National Comprehensive Cancer Network suggests that pancreatitis is a criterion for permanent discontinuation, with an incidence of 1-2% of clinical pancreatitis grade ≥3 5, 6
- For asymptomatic elevations (chemical pancreatitis), continued treatment with close monitoring is possible 5, 6
