Nitroglycerin Use in Acute Coronary Syndrome
Indications and Dosage
- The American College of Cardiology recommends initiating intravenous nitroglycerin at 10 μg/min and titrating every 3-5 minutes based on pain relief and hemodynamic tolerance, for patients with persistent anginal pain after sublingual nitrates, or when acute coronary syndrome is accompanied by hypertension or pulmonary edema 1, 2, 3
- Consider intravenous nitroglycerin in patients with persistent anginal pain after 3 doses of sublingual nitroglycerin (0.3-0.4 mg every 5 minutes) 1, 2, 3
- The American Heart Association suggests intravenous nitroglycerin for acute coronary syndrome accompanied by hypertension or pulmonary edema 1, 2, 3
- High-risk patients without hypotension may also benefit from intravenous nitroglycerin 4
Contraindications
- The American College of Cardiology advises against administering nitroglycerin in patients with systolic blood pressure <90 mmHg or a drop >30 mmHg from baseline 1, 2, 3, 5
- Suspected right ventricular infarction is a contraindication for nitroglycerin 1, 2, 3
- Recent use of phosphodiesterase-5 inhibitors (within 12 hours of avanafil, 24 hours of sildenafil or vardenafil, or 48 hours of tadalafil) is a contraindication for nitroglycerin 1, 2, 3, 6
Monitoring and Titration
- The American College of Cardiology recommends maintaining systolic blood pressure ≥90 mmHg and avoiding a drop >30 mmHg from baseline 1, 2, 3
- Titrate intravenous nitroglycerin in increments of 5-10 μg/min every 3-5 minutes until symptomatic or hemodynamic response is achieved 1, 2, 4
Tolerance and Alternative Therapies
- Tolerance to nitroglycerin may occur after approximately 24 hours of continuous therapy, requiring periodic increases in infusion rate to maintain efficacy 1, 2, 3
- Consider intermittent dosing regimens to prevent tolerance in prolonged therapy 4, 6
- If nitroglycerin is contraindicated, alternative analgesic options include morphine IV 2-4 mg (repeat every 5-15 minutes as needed) or fentanyl IV 25-50 μg (repeat as needed) 1, 2, 3
