Medication Therapy Management for Amlodipine and Metoprolol

Introduction to Medication Therapy Management

  • The American Heart Association recommends beta-blockers for patients with a history of MI, heart failure with reduced ejection fraction, or angina 1

Patient Assessment and Monitoring

  • The American College of Cardiology recommends monitoring for additive effects of the combination of beta-blockers and calcium channel blockers, including signs of excessive bradycardia, hypotension, or heart block 1
  • Special caution is needed in patients with risk factors for cardiogenic shock, including age >70 years, systolic BP <120 mmHg, heart rate >110 bpm or <60 bpm, as recommended by the American College of Cardiology 3
  • Monitoring of vital signs, including heart rate and blood pressure, is necessary to minimize adverse effects 4

Medication Management

  • The American Heart Association recommends metoprolol for patients with a history of MI, heart failure with reduced ejection fraction, or angina, with contraindications including severe bradycardia, heart block, cardiogenic shock, and decompensated heart failure 1
  • Metoprolol should be started at low doses and gradually titrated in patients with heart failure, as recommended by the American Heart Association 2
  • Amlodipine can be taken without regard to meals, and metoprolol should be taken consistently with or without food, with advice against abrupt discontinuation to prevent rebound hypertension or worsening angina 4

Patient Education and Adherence

  • Patients should be educated on warning signs requiring medical attention, including symptoms of hypotension, signs of heart failure, and chest pain or palpitations, as recommended by the American College of Cardiology 4
  • Home blood pressure monitoring techniques should be taught, including proper timing, technique, and record keeping, to help patients manage their condition effectively 4
