Dobutamine Dosing and Administration Guidelines
Adult Initiation and Titration
- Begin continuous IV infusion of dobutamine at 0.5–1.0 µg/kg/min (FDA label) or 2–3 µg/kg/min as recommended by the European Society of Cardiology (ESC); no loading bolus is used. 1
- Titrate by doubling the dose every 15 minutes according to hemodynamic response, tolerability, and clinical signs of improved perfusion (urine output > 100 mL/h in the first 2 h, warming of extremities, improved mentation). 1
- Increase infusion in 2 µg/kg/min increments during titration. 1
- The usual effective range is 2–20 µg/kg/min; doses > 20 µg/kg/min are rarely required. 1
- Rarely, doses up to 40–50 µg/kg/min have been needed (e.g., refractory shock). Class IIb, Level C evidence (ESC). 1, 2
- Patients receiving β‑blockers may require up to 20 µg/kg/min to overcome receptor blockade. 1
Dose‑Response Endpoints and Clinical Targets
- Titration is typically limited by excessive tachycardia, arrhythmias, or myocardial ischemia before reaching the maximum dose. 1
- Desired hemodynamic endpoints include:
Pediatric Dosing (Rule of 6)
- For children, the “Rule of 6” calculates the infusion: 0.6 × body weight (kg) = milligrams of dobutamine diluted to 100 mL saline; delivering 1 mL/h provides 0.1 µg/kg/min. Class IIb (no explicit level given). 3
Hemodynamic Indications
- Dobutamine is indicated for patients with objective low cardiac output and hypoperfusion (cold/clammy skin, oliguria < 20 mL/h, altered mentation, metabolic acidosis) or systolic blood pressure < 85–90 mm Hg despite adequate diuretics and vasodilators. Class IIb, Level C (ESC). 1, 4
Blood‑Pressure‑Guided Algorithm (ESC)
| SBP (mm Hg) | Recommendation |
|---|---|
| > 100–110 | Use vasodilators and diuretics; dobutamine not indicated. [1] |
| 85–100 | Start diuretics; add dobutamine only if response is inadequate and hypoperfusion persists. [1] |
| < 85 (shock) | Consider dobutamine when pulmonary congestion predominates and cardiac output is low. [1, 4] |
Pharmacological Stress Testing
- For dobutamine stress echocardiography or nuclear imaging, start at 5 µg/kg/min, increase to 10 µg/kg/min, then escalate by 10 µg/kg/min every 3 minutes up to a maximum of 40–50 µg/kg/min or until a predefined endpoint. Class IIb (no level). 2
- Target heart rate: 85 % of age‑predicted maximum (or 70 % for submaximal protocols). 2
- Radioisotope injection is performed at peak dose; the infusion continues for 1 minute post‑injection. 2
- Adjunctive atropine (up to 1 mg IV) may be added if the target heart rate is not achieved. 2
- Endpoints include new/worsening wall‑motion abnormalities, attainment of target heart rate, worsening arrhythmia, moderate angina, intolerable side effects, or significant blood‑pressure changes. 2
Monitoring Requirements
- Continuous ECG monitoring and frequent blood‑pressure measurement (preferably via arterial catheter) are mandatory during infusion. Class IIb, Level C (ESC) and weak recommendation (Surviving Sepsis Campaign). 1, 4
- Arrhythmia surveillance: ventricular ectopy occurs in 3–15 % of patients. 1
- Myocardial‑ischemia monitoring is required in patients with known coronary artery disease. 1, 2
Weaning and Discontinuation
- Abrupt discontinuation may cause hypotension, recurrent congestion, or renal insufficiency; therefore, taper by decreasing 2 µg/kg/min steps while optimizing oral vasodilators. Class IIb, Level C (ESC). 1
- Tolerance to dobutamine may develop after 24–48 hours of continuous infusion. 1
- Reassessment of the need for inotropic support should occur after 48–72 hours. 1
Critical Safety Warnings
- Dobutamine increases the risk of tachycardia, arrhythmias, and myocardial ischemia; it should not be initiated solely on low blood pressure without confirming objective hypoperfusion. Class IIb, Level C (ESC). 1
- In atrial fibrillation, dobutamine can facilitate rapid AV conduction. 1
- It may provoke chest pain and enlarge infarct size in patients with coronary artery disease. 1
- Prolonged infusion (> 48–72 hours) raises the risk of tolerance. 1
Evidence Quality Summary
- The European Society of Cardiology assigns Class IIb, Level C evidence to dobutamine use in acute heart failure, reflecting the absence of controlled mortality trials and data suggesting increased cardiovascular events. 1
- The Surviving Sepsis Campaign (2016) recommends dobutamine up to 20 µg/kg/min for patients with persistent hypoperfusion despite adequate fluid resuscitation and vasopressor therapy (weak recommendation, low‑quality evidence). 4
