Management of Severe Hypertension with Labetalol
Blood Pressure Targets
- For malignant hypertension, target a 20-25% reduction in mean arterial pressure over several hours, as recommended by the European Society of Cardiology 1
- For hypertensive encephalopathy, aim for immediate reduction of mean arterial pressure by 20-25%, as recommended by the European Society of Cardiology 1
- For acute ischemic stroke with BP >220/120 mmHg, target a 15% reduction in mean arterial pressure, as recommended by the European Society of Cardiology 1
- For acute hemorrhagic stroke, aim for systolic BP between 130-180 mmHg, as recommended by the European Society of Cardiology 1
- For eclampsia/pre-eclampsia, target systolic BP <160 mmHg and diastolic BP <105 mmHg, as recommended by the European Society of Cardiology 1
Clinical Considerations
- Labetalol is particularly useful in hypertensive encephalopathy as it preserves cerebral blood flow better than nitroprusside, as reported by the European Society of Cardiology 1
- Excessive BP reduction (>50% decrease in mean arterial pressure) should be avoided as it has been associated with ischemic stroke and death, as reported by the European Society of Cardiology 1
- Labetalol is contraindicated in patients with second or third-degree heart block, bradycardia, and decompensated heart failure 2
- Labetalol should be used with caution in patients with reactive airways disease (asthma, COPD) 3
