Dulaglutide Dosing and Clinical Considerations

Adult Dosing and Renal Impairment

  • No dose adjustment is required for patients with renal impairment, including those with end-stage renal disease, according to the Endocrine Society and Kidney International 1, 2, 3
  • The American College of Cardiology recommends that dulaglutide can be used in patients with any level of renal impairment without dose adjustment 1, 3, 4

Special Populations and Clinical Considerations

  • The American College of Cardiology suggests that dulaglutide has demonstrated cardiovascular benefit in reducing major adverse cardiovascular events in adults with type 2 diabetes with established cardiovascular disease or multiple risk factors, with a strength of evidence based on high-quality trials 6, 7
  • The American Diabetes Association recommends that when adding dulaglutide to sulfonylureas or insulin, consider reducing doses of these agents to minimize hypoglycemia risk, based on evidence from a 2025 study 8

Combination Therapy and Renal Benefits

  • The American College of Cardiology recommends up-titrating dulaglutide slowly to reduce gastrointestinal side effects, with a strength of evidence based on high-quality trials 6, 7
