Post-ERCP Management of Biliary Pancreatitis with CBD Stone

Immediate Post-ERCP Monitoring

  • The American College of Gastroenterology recommends monitoring liver function tests and pancreatic enzymes daily until normalized after ERCP for biliary pancreatitis with CBD stone removal, and performing transabdominal ultrasound to confirm duct decompression 1, 2
  • The American College of Radiology suggests checking complete blood count and coagulation parameters to monitor for post-sphincterotomy bleeding complications 2, 3

Imaging Evaluation

  • Transabdominal ultrasound should be performed within 24-48 hours post-ERCP to confirm common bile duct decompression and assess for residual ductal dilatation, with a normal bile duct diameter being less than 6mm 3, 5

Clinical Surveillance for Complications

  • The American College of Radiology recommends observing for worsening abdominal pain, fever, or rising pancreatic enzymes in the first 24-48 hours, as post-ERCP pancreatitis occurs in 4-5.2% of cases 5, 6
  • Any clinical deterioration warrants repeat imaging with CT scan to assess for pancreatic necrosis or fluid collections 7

Definitive Management Planning

  • The American College of Surgeons recommends scheduling laparoscopic cholecystectomy within 2 weeks of presentation, preferably during the same hospital admission, to prevent recurrent biliary pancreatitis 1, 2
  • Delaying cholecystectomy beyond 4 weeks significantly increases the risk of recurrent pancreatitis, which occurs in a substantial proportion of patients if the gallbladder remains in situ 8

Special Considerations and Pitfalls

  • For patients deemed unsuitable for cholecystectomy due to medical comorbidities, biliary sphincterotomy with complete duct clearance alone is an acceptable alternative, though this carries a 5% risk of recurrent primary CBD stone formation 3, 6
  • In severe acute pancreatitis, defer cholecystectomy until the inflammatory process has completely subsided and any local complications have resolved, with CT scanning performed 3-10 days after presentation to assess for pancreatic necrosis 7, 8
