Rituximab Subcutaneous Dosing and Administration Guidelines

Patient Selection and Dosing

  • All patients must receive at least one full IV dose of rituximab 375 mg/m² before switching to SC administration, ensuring tolerability and establishing baseline exposure, as recommended by the European Society for Medical Oncology 1
  • The American Society of Hematology recommends that prophylaxis against herpes zoster is strongly recommended when rituximab is combined with proteasome inhibitors, due to increased infection risk 2, 3
  • The National Comprehensive Cancer Network suggests that antibacterial/antiviral prophylaxis should be considered when rituximab is combined with bendamustine due to prolonged T-cell suppression 1

Special Populations

  • The International Society of Geriatric Oncology recommends that SC rituximab is particularly advantageous for elderly vulnerable patients who cannot tolerate prolonged IV infusions, as it avoids the need for IV access in patients with difficult venous access 1
  • The European Hematology Association suggests that in patients with IgM ≥4 g/dL, consider preemptive plasmapheresis before rituximab to avoid symptomatic IgM flare, and weekly or SC administration of bortezomib is preferred when combined with rituximab 4, 5

Administration Technique

  • The American College of Physicians recommends that SC rituximab should be administered via SC injection after the first IV infusion is tolerated, with administration time approximately 5-7 minutes for SC vs 90-263 minutes for IV 1
