Nitroglycerin Administration for Chest Pain

Initial Management

  • The American Heart Association recommends that patients take one sublingual nitroglycerin tablet (0.3-0.4 mg) at the first sign of chest pain, and may take up to 3 doses (one every 5 minutes) if symptoms persist 1, 2
  • For patients with chronic stable angina, the American College of Cardiology suggests that if symptoms significantly improve after the first nitroglycerin dose, they may still take up to 3 doses (one every 5 minutes) 3, 4

Dosage Considerations

  • The standard sublingual nitroglycerin dosage is 0.3-0.4 mg per tablet, with a maximum recommended total of 3 doses (0.9-1.2 mg total) per episode 1, 2

Contraindications and Precautions

  • Nitroglycerin should not be administered to patients with systolic blood pressure less than 90 mmHg or ≥30 mmHg below baseline, severe bradycardia (less than 50 bpm), or suspected right ventricular infarction 2
  • The American Heart Association also recommends that nitroglycerin should not be administered to patients with recent use of phosphodiesterase inhibitors (within 24-48 hours), although this is based on a guideline from Praxis Medical Insights, an alternative source such as the American College of Cardiology states that nitroglycerin is contraindicated in patients with severe hypotension or those who have taken phosphodiesterase inhibitors within the last 24-48 hours 5

Transition to IV Nitroglycerin

  • Intravenous nitroglycerin is indicated for relief of ongoing ischemic discomfort, control of hypertension, and management of pulmonary congestion 1, 2
