Fluconazole Dosing in DKA Patients with Impaired Renal Function

Dosing Considerations

  • The Infectious Diseases Society of America recommends a fluconazole dose of 200 mg (3 mg/kg) daily for 2 weeks for fluconazole-susceptible Candida species in patients with candiduria/urinary tract infections, with a strength of evidence based on clinical guidelines 1, 2
  • For oropharyngeal candidiasis, the recommended dose is 100-200 mg daily for 7-14 days, with a moderate to severe disease requiring the same dose range 1

Renal Function and Dosing

  • In patients with impaired renal function, the dose should be adjusted based on creatinine clearance, with a reduced maintenance dose of 50% for patients with CrCl ≤50 mL/min, as suggested by clinical practice guidelines 3

Specific Indications

  • For cystitis, the recommended dose is 200 mg daily for 2 weeks, while for pyelonephritis, the dose is 200-400 mg (3-6 mg/kg) daily for 2 weeks, based on clinical evidence 2
