Pulmonary Sarcoidosis Flare Management

Initial Treatment

  • The European Respiratory Society recommends starting oral prednisone 20-40 mg daily for 3-6 months as first-line treatment for a pulmonary sarcoidosis flare, then tapering to the lowest effective dose 1, 2
  • Treatment is indicated if there is risk of mortality, permanent organ disability, or significant quality of life impairment, according to the European Respiratory Society 3, 4
  • Nearly half of sarcoidosis patients never require systemic treatment due to spontaneous resolution, so asymptomatic patients without organ dysfunction risk should not be treated, as suggested by the European Respiratory Society 4, 5

Initial Prednisone Dosing Protocol

  • Start prednisone 20-40 mg daily for symptomatic disease with organ dysfunction risk, as recommended by the European Respiratory Society 1, 2, 3
  • Reduce the starting dose in patients with diabetes, psychosis, or osteoporosis, according to the European Respiratory Society 3, 4
  • Continue the initial dose for 3-6 months to assess therapeutic response, as suggested by the European Respiratory Society 1, 5

Monitoring Response and Tapering

  • Evaluate with clinical assessment, pulmonary function testing, and chest imaging at 3 months, as recommended by the European Respiratory Society 3
  • If improved, begin tapering prednisone gradually to the lowest dose that maintains symptom control and disease stability, according to the European Respiratory Society 1, 5
  • Target a maintenance dose ≤10 mg daily or discontinuation if possible, as suggested by the European Respiratory Society 1

Second-Line Therapy

  • Add methotrexate 10-15 mg weekly if disease progresses or no improvement is seen, as recommended by the European Respiratory Society 1, 2
  • Methotrexate is the preferred second-line agent based on the most extensive evidence and best tolerability profile, according to the European Respiratory Society 1, 4

Third-Line Treatment for Refractory Disease

  • Add infliximab 3-5 mg/kg initially, at 2 weeks, then every 4-6 weeks for patients with continued disease despite glucocorticoids and methotrexate, as recommended by the European Respiratory Society 1, 2
  • Infliximab is particularly effective for severe manifestations including cardiac and neurologic sarcoidosis, according to the European Respiratory Society 3

Supportive Care

  • Provide calcium and vitamin D supplementation during prolonged steroid use, as recommended by the European Respiratory Society 3
  • Monitor blood pressure, serum glucose, and bone density due to cumulative glucocorticoid toxicity, according to the European Respiratory Society 2
