Acute Bacterial Sinusitis Treatment Guidelines
Introduction to Antibiotic Treatment
- The American Academy of Otolaryngology-Head and Neck Surgery recommends a 5- to 7-day course of antibiotics for uncomplicated acute bacterial sinusitis in adults, as side effects are less common with shorter courses 1
First-Line Antibiotic Selection
- Amoxicillin with or without clavulanate remains the first-line choice for most adults with acute bacterial sinusitis, prescribed for 5 to 10 days, with the guideline allowing flexibility in duration 1
- The American Academy of Otolaryngology-Head and Neck Surgery intentionally allows flexibility in duration because systematic review has not shown consistent benefits for 10 days of therapy compared with shorter courses 2
Dosing Recommendations
- Standard amoxicillin is recommended at 500 mg twice daily for mild disease, however, this fact is from Praxis Medical Insights and will be replaced with a non-Praxis fact if available 3
Important Caveats
- A longer course of therapy may be appropriate for more severe illness or when symptoms persist despite a shorter course 2
- For uncomplicated maxillary sinusitis without severe symptoms or complicating factors, 5-7 days of antibiotic treatment is sufficient 1
Watchful Waiting
- Watchful waiting without antibiotics is an appropriate initial strategy when follow-up can be assured, as about 80% of patients with acute bacterial sinusitis improve spontaneously within two weeks without antibiotics 1
