Anticoagulation in Valvular Atrial Fibrillation

Introduction to Valvular Atrial Fibrillation

  • The American College of Cardiology recommends that rivaroxaban is contraindicated in patients with valvular atrial fibrillation, which includes those with calcified mitral valve disease, and warfarin remains the only evidence-based anticoagulant for this population 1, 2, 3
  • The FDA approval for rivaroxaban explicitly states it is indicated only for "nonvalvular atrial fibrillation" to reduce stroke and systemic embolism risk, with the term "nonvalvular" excluding patients with mitral stenosis, prosthetic heart valves, and hemodynamically significant valve disease 1, 2, 3

Anticoagulation Management

  • The American College of Cardiology and American family physician recommend using dose-adjusted warfarin (target INR 2.0-3.0) as the standard of care for patients with calcified mitral valve disease and atrial fibrillation 1, 2, 3, 4
  • The American family physician suggests monitoring INR regularly (typically weekly initially, then monthly once stable) and adjusting the dose based on INR results and clinical factors 4

Special Considerations

  • Aspirin alone may be considered if warfarin is contraindicated or ineffective, though it carries significantly less efficacy 4
  • Aspirin plus clopidogrel is not recommended as an alternative due to similar bleeding risk to warfarin 4
  • Left atrial appendage closure devices may be considered in select cases where anticoagulation is absolutely contraindicated 1, 2

Assessment and Treatment

  • The American College of Cardiology recommends assessing renal function before any anticoagulant, as rivaroxaban is contraindicated in severe renal impairment (CrCl <15 mL/min) regardless of valve status 1, 2, 3, 5
  • An echocardiogram should be obtained to assess valve area, gradient, and regurgitation severity in patients with mitral valve calcification 6
