Ciprofloxacin Dosing Considerations

Special Clinical Contexts

  • For NTM pulmonary disease, doses of 500-750 mg twice daily orally have been used, though ciprofloxacin is not licensed for this indication in the UK, as recommended by the British Thoracic Society 1
  • For disseminated M. avium complex disease, 750 mg twice daily is recommended, according to the Infectious Diseases Society of America 2

Monitoring Requirements

Baseline and Ongoing Monitoring

  • ECG monitoring should be performed at baseline, repeated at 2 weeks, and after adding any QT-prolonging medications due to risk of QTc prolongation, as suggested by the American Heart Association 1
  • Blood glucose should be monitored regularly in diabetic patients due to hypoglycemia risk, as advised by the American Diabetes Association 1
  • Routine labs, including CBC, renal function, and liver function tests, should be performed intermittently throughout treatment, as recommended by the National Kidney Foundation 1

Therapeutic Drug Monitoring

  • Drug level monitoring may be considered in specific situations, such as known or suspected malabsorption, poor treatment response, with target trough levels of 1-1.5 mg/L and target peak levels of 3-4.5 mg/L, as suggested by the Clinical and Laboratory Standards Institute 1
