Vancomycin Monitoring in Patients on CRRT

Introduction to Vancomycin Monitoring

  • Intensive drug monitoring for vancomycin is essential in patients on CRRT, as acute renal injury risk remains a critical concern that requires frequent trough level monitoring despite renal replacement therapy, according to the Clinical Infectious Diseases guidelines 1, 2

Nephrotoxicity Risk and Monitoring

  • The presence of CRRT does not eliminate the risk of vancomycin-induced nephrotoxicity, and these patients are at higher risk due to their underlying acute kidney injury and critical illness, as stated by the Clinical Infectious Diseases guidelines 1, 2
  • Sustained trough concentrations >20 μg/mL significantly increase nephrotoxicity risk even in patients on CRRT, as reported by Praxis Medical Insights 2, 3
  • Vancomycin-induced nephrotoxicity is defined as multiple increases in serum creatinine of 0.5 mg/dL or 150% increase from baseline, according to the Clinical Infectious Diseases guidelines 1, 2

Specific Monitoring and Dosing Protocols

  • Target trough levels of 15-20 mg/L for serious infections, such as bacteremia, endocarditis, osteomyelitis, and pneumonia, as recommended by the Clinical Infectious Diseases guidelines 1, 2
  • Monitor serum creatinine at least twice weekly throughout therapy, as suggested by Praxis Medical Insights 4
  • A loading dose of 25-35 mg/kg is recommended to rapidly achieve therapeutic levels, according to Praxis Medical Insights 4

Management of Elevated Levels

  • If trough levels exceed 20 mg/L despite CRRT, hold the next scheduled dose immediately and recheck trough level before administering subsequent doses, as advised by Praxis Medical Insights 2, 3
