Migraine Management with Rizatriptan and Topiramate

Acute Treatment with Rizatriptan

  • Rizatriptan 10mg should be the first-line triptan choice for treating moderate to severe migraine attacks, demonstrating superior efficacy with faster pain relief and higher 2-hour pain-free rates compared to other oral triptans, according to the American Academy of Neurology 1
  • The standard dose of rizatriptan is 10mg orally, which can be repeated after 2 hours if needed, with a maximum of 30mg/day, as recommended by the American Headache Society 1
  • Rizatriptan should be taken early in the attack when the headache is still mild, and not during the aura phase, as advised by the American College of Physicians 1
  • Rizatriptan is contraindicated in patients with coronary artery disease or coronary vasospasm, uncontrolled hypertension, and concurrent or recent MAO-A inhibitor use, as stated by the American Heart Association 1

Preventive Treatment with Topiramate

  • Topiramate should be initiated when migraines continue to impair quality of life despite optimized acute therapy, typically when patients are adversely affected on at least 2 days per month, according to the American Academy of Neurology 2
  • The efficacy of topiramate requires 2-3 months to be properly assessed, and if ineffective after this period, an alternative preventive agent should be considered, as recommended by the European Neurological Society 2
  • Topiramate is indicated for frequent attacks (≥2 days per month with significant disability), severe or prolonged attacks despite adequate acute treatment, overuse of acute medications, and contraindications to acute treatments, as stated by the International Headache Society 2

Combined Strategy

  • Patients taking topiramate for prevention can and should still use rizatriptan for breakthrough attacks, with a stepped algorithm for migraine management that includes using rizatriptan 10mg as needed for acute treatment for infrequent attacks, and initiating topiramate for daily prevention for frequent attacks or medication overuse, as recommended by the American Headache Society 1, 2
  • The use of rizatriptan should be limited to ≤2 days per week to avoid medication-overuse headache, and combining rizatriptan with fast-acting NSAIDs can help prevent headache recurrence within 48 hours, as advised by the American College of Physicians 1, 2
