Herb-Drug Interactions and Cholesterol Levels

Introduction to St. John's Wort and Cholesterol

  • St. John's Wort is a strong inducer of cytochrome P450 3A4 (CYP3A4) enzymes and P-glycoprotein (P-gp), affecting cholesterol levels and drug metabolism, as reported by the European Heart Journal 1, 2

Effects on Statin Effectiveness

  • The American Heart Association recommends avoiding St. John's Wort in patients with hypercholesterolemia, especially those taking statins, as it can reduce statin effectiveness by accelerating their metabolism through CYP3A4 induction 1
  • Concomitant use of St. John's Wort with statins can increase cholesterol levels by reducing statin effectiveness through induction of CYP450 enzymes 1

Drug Interactions

  • St. John's Wort has numerous drug interactions, including reducing the effectiveness of oral anticoagulants, as recommended by the American College of Cardiology 3
  • St. John's Wort decreases serum levels of digoxin, according to the Annals of Neurology 4
  • The National Comprehensive Cancer Network recommends considering alternative treatments due to St. John's Wort's interactions with medications metabolized by CYP3A4 5

Clinical Guidance

  • Clinicians should monitor lipid profiles more frequently in patients taking St. John's Wort and consider alternative treatments for depression in patients with cardiovascular risk factors or hypercholesterolemia, as suggested by the American Heart Association 1
  • St. John's Wort should be discontinued at least 2 weeks before surgery or when stopping use, due to its long-acting constituents and effects on drug metabolism, as recommended by the Mayo Clinic Proceedings and the American Society for Perioperative Assessment and Quality Improvement (SPAQI) 1
  • A 2-week washout period should be allowed when switching to medications with potential interactions, especially antidepressants, anticoagulants, or immunosuppressants, to minimize the risk of adverse interactions 1
  • After discontinuation, the body requires time to restore normal enzyme activity levels and eliminate active constituents, allowing for safer medication management and reduced risk of adverse interactions 1
Medication Effect of St. John's Wort
Statins Reduces effectiveness by accelerating metabolism
Oral anticoagulants Reduces effectiveness
Digoxin Decreases serum levels
Medications metabolized by CYP3A4 Interacts with St. John's Wort, consider alternative treatments
