Guidelines for Restarting Adderall After Prolonged Discontinuation

Initial Dosing and Titration Protocol

  • The American Academy of Child and Adolescent Psychiatry (AACAP) advises that after a ≥5‑month interruption, clinicians should begin with a low dose of 5 mg once or twice daily and titrate upward over 2–4 weeks to the prior therapeutic dose, thereby reducing adverse‑effect risk and permitting reassessment of the optimal dose. 1
  • AACAP’s FDA‑based titration recommendation for patients ≥ 6 years old is to start at 5 mg once or twice daily, increase by 5 mg increments at weekly intervals until the desired clinical response is achieved; this protocol applies equally to treatment‑naïve individuals and those restarting therapy. 2
  • Week 1: 5 mg once or twice daily (total 5–10 mg/day). 2
  • Week 2: If tolerated, increase to 10 mg once or twice daily (total 10–20 mg/day). 2
  • Weeks 3–4: Continue 5‑mg weekly increments until the previous effective dose of 20 mg/day is reached. 2, 3

Monitoring and Safety Assessments

  • Conduct weekly evaluations of ADHD symptom control and side‑effect profile during each dose adjustment. 2, 3
  • Maintain telephone contact with the patient (or caregiver) throughout the titration phase to promptly address concerns. 2
  • At every dose increase, record vital signs (blood pressure, pulse) and body weight. 1
  • Systematically screen for common stimulant‑related adverse effects—including insomnia, reduced appetite, headache, anxiety, and cardiovascular changes. 3

Clinical Considerations for Dose Optimization

  • After a ≥5‑month gap, the patient’s clinical status may have changed, making the former 20‑mg dose potentially sub‑optimal (either too high or too low). 2
  • Re‑titration offers an opportunity to identify the true optimal dose rather than assuming the previous dose was appropriate. 4

Best Practices and Pitfalls to Avoid

  • Do not restart at the full 20 mg dose immediately; this contravenes AACAP/FDA guidance and heightens the risk of adverse events. 2, 3
  • Avoid rapid titration; a 2–4‑week titration schedule is the standard to ensure safety and efficacy. 2, 3
  • Do not presume the prior dose was optimal; use the re‑titration period to fine‑tune therapy based on current response. 1, 4
  • Do not neglect weekly side‑effect monitoring, as early detection of problems is essential for safe dose escalation. 2, 3
