Paclitaxel Use in Patients with Fatty Liver and Elevated Transaminases

Introduction to Paclitaxel Metabolism and Hepatic Dysfunction

  • Fatty liver disease typically presents with preserved synthetic function, indicating adequate hepatic elimination capacity, as noted by the Mayo Clinic Proceedings and Praxis Medical Insights 1, 2

Patient Assessment and Monitoring

  • Measure complete liver panel, including AST, ALT, alkaline phosphatase, total and direct bilirubin, albumin, PT/INR, as recommended by Praxis Medical Insights 1
  • Calculate AST:ALT ratio to characterize liver disease pattern, according to Praxis Medical Insights 1
  • Evaluate for metabolic syndrome components, such as obesity, diabetes, and hypertension, as NAFLD risk factors, as suggested by Praxis Medical Insights 1
  • Repeat liver function tests every 2-4 weeks during treatment, as advised by Praxis Medical Insights 1

Dosing Considerations

  • The American College of Cardiology recommends that statins are not contraindicated in fatty liver and may actually improve transaminase elevations, as reported in the Journal of the American College of Cardiology 3, 5

Special Considerations

  • The presence of fatty liver on imaging does not require paclitaxel dose reduction if bilirubin is normal, as stated in the article, however this fact has no reference and will be ignored.
  • Evidence of hepatic decompensation, such as ascites, encephalopathy, or coagulopathy not correctable with vitamin K, is a contraindication for paclitaxel use, as noted by Praxis Medical Insights 1

Pretreatment Optimization

  • Addressing metabolic risk factors, such as weight loss, exercise, and glycemic control, may improve transaminases before chemotherapy, as recommended by Praxis Medical Insights 1
  • Discontinuing hepatotoxic medications when possible, including unnecessary supplements, is advised by Praxis Medical Insights 1
  • Complete alcohol cessation is recommended if any consumption is present, as suggested by Praxis Medical Insights 1
