Diagnostic Considerations for Atypical Chronic Lymphocytic Leukemia
Key Diagnostic Features
- The National Comprehensive Cancer Network recommends that CD5+, CD19+, CD20+, and CD200+ expression, along with lambda light chain restriction, are characteristic of chronic lymphocytic leukemia (CLL) or small lymphocytic lymphoma (SLL) 1, 2, 3
- The absence of CD23 expression is unusual in CLL/SLL and requires careful exclusion of mantle cell lymphoma (MCL) 1, 2, 4
- CD200 positivity strongly argues against MCL, as MCL is typically CD200- 7, 6
Diagnostic Algorithm
- The National Comprehensive Cancer Network suggests that cyclin D1 expression by flow cytometry or immunohistochemistry, and FISH for t(11;14), are mandatory to exclude MCL 1, 2, 4, 5
- Evaluation of CD200 expression is also necessary, as it favors CLL over MCL 7, 6
- If cyclin D1 is negative but there is high clinical suspicion of MCL, SOX11 immunohistochemistry should be considered to exclude rare cyclin D1-negative MCL 5, 6
Prognostic Testing for CLL/SLL
- The National Comprehensive Cancer Network recommends obtaining prognostic markers, including FISH panel, TP53 mutation analysis, IGHV mutation status, and stimulated karyotype, once MCL is excluded 1, 2, 4
Clinical Management
- If cyclin D1 and t(11;14) are negative, confirming atypical CLL/SLL, the National Comprehensive Cancer Network suggests staging using Rai or Binet staging systems, and observation is appropriate for asymptomatic patients with low tumor burden 3, 1, 2
- Prognostic markers guide treatment selection when therapy is indicated, and repeat FISH and karyotype at the time of treatment as cytogenetic abnormalities evolve 1, 4
