Treatment Recommendations for Breakthrough Bleeding with Nexplanon

Initial Assessment

  • The Centers for Disease Control and Prevention recommends ruling out underlying gynecological problems such as medication interactions, STDs, pregnancy, or uterine pathology before initiating treatment for breakthrough bleeding with Nexplanon 1
  • Approximately 22% of patients experience amenorrhea, 34% experience infrequent spotting, 7% report frequent bleeding, and 18% report prolonged bleeding with Nexplanon, according to the Centers for Disease Control and Prevention 2, 1
  • The Centers for Disease Control and Prevention reassures patients that these bleeding irregularities are generally not harmful 1

Treatment Algorithm for Breakthrough Bleeding

  • The American College of Obstetricians and Gynecologists recommends NSAIDs for short-term treatment (5-7 days) for breakthrough bleeding with Nexplanon 1
  • Celecoxib 200 mg daily for 5 days has shown significant cessation of bleeding within 7 days, as reported by the Centers for Disease Control and Prevention 2
  • Mefenamic acid 500 mg three times daily for 5 days also demonstrates significant bleeding cessation within 7 days, according to the Centers for Disease Control and Prevention 2
  • The American College of Obstetricians and Gynecologists recommends hormonal treatment with low-dose combined oral contraceptives or estrogen for short-term treatment (10-20 days) if medically eligible for patients with breakthrough bleeding with Nexplanon 1

Special Considerations

  • Ibuprofen has shown inconsistent results in clinical trials for treating breakthrough bleeding with Nexplanon, with one study showing reduction in bleeding days while another showed no significant difference compared to placebo, as reported by the Centers for Disease Control and Prevention 2
  • Vitamin E has shown mixed results, with one small study showing reduced bleeding days but a larger study showing no significant differences, according to the Centers for Disease Control and Prevention 2
  • Aspirin did not show significant improvement in bleeding patterns for patients with breakthrough bleeding with Nexplanon, as reported by the Centers for Disease Control and Prevention 2

Follow-up Recommendations

  • If irregular bleeding persists despite treatment and the woman finds it unacceptable, the American College of Obstetricians and Gynecologists recommends counseling her on alternative contraceptive methods 1
  • Enhanced counseling about expected bleeding patterns has been shown to reduce discontinuation rates in clinical trials, as reported by the Centers for Disease Control and Prevention 2, 1
  • For patients who wish to continue with Nexplanon despite breakthrough bleeding, treatments can be repeated as needed for symptom management, according to the Centers for Disease Control and Prevention 1

Important Caveats

  • Treatment is focused on temporary symptom management rather than permanent correction of the bleeding pattern for patients with breakthrough bleeding with Nexplanon, as reported by the Centers for Disease Control and Prevention 2, 1
  • If bleeding is heavy or prolonged and persists despite treatment, the American College of Obstetricians and Gynecologists recommends considering removal of the implant and switching to an alternative contraceptive method 1
