Management of SIADH

Initial Assessment and Diagnosis

  • The diagnosis of SIADH should be confirmed by documenting hyponatremia (serum sodium < 134 mEq/L) with plasma osmolality < 275 mOsm/kg, inappropriately concentrated urine (urine osmolality > 100-500 mOsm/kg) with urine sodium > 20 mEq/L, and a euvolemic state, according to Praxis Medical Insights 4
  • Normal thyroid and adrenal function should be confirmed to exclude other causes of hyponatremia, as recommended by Praxis Medical Insights 4, 5

Treatment Algorithm

  • For an 8 kg child with mild or asymptomatic hyponatremia, fluid restriction to 65-80% of calculated maintenance (approximately 520-650 mL/day) is recommended, using isotonic saline if IV fluids are needed, as per Intensive Care Medicine 1, 2, 3
  • The American Academy of Pediatrics recommends using isotonic fluids (0.9% NaCl with appropriate dextrose and potassium) if IV maintenance is required, to avoid worsening hyponatremia 1, 2
  • The goal of treatment is to increase sodium levels by 6 mmol/L over 6 hours or until severe symptoms resolve, without exceeding 8 mmol/L in 24 hours, as recommended by Praxis Medical Insights 4, 5, 6

Critical Monitoring Parameters

  • Daily reassessment of serum sodium, fluid balance, clinical status, and urine output and osmolality is mandatory, as recommended by Intensive Care Medicine 1, 2, 3
  • Serum sodium should be checked at least daily, and more frequently during active correction, to prevent overcorrection, as per Intensive Care Medicine 1, 2, 3

Pharmacological Options

  • Demeclocycline may be considered as a second-line agent for refractory cases, although it has limited pediatric data, as noted by Praxis Medical Insights 4

Special Considerations

  • The sodium threshold for seizures in pediatric patients is 120 mmol/L, above which seizures become likely, as recommended by Praxis Medical Insights 5
