Dosage and Administration of Salbutamol and Spiriva

Salbutamol (Albuterol) Dosage

  • The European Respiratory Society recommends a typical dose of salbutamol via inhaler with a spacer is 200-400 μg (2-4 puffs) every 4-6 hours as needed, for mild symptoms 1
  • For moderate symptoms, the dose can be increased up to 1000 μg (10 puffs) every 4-6 hours as needed, with a maximum of 8-12 puffs per 24 hours for routine use 1
  • The European Respiratory Review suggests that for acute symptoms, 2.5-5.0 mg of salbutamol can be administered via nebulizer every 20 minutes for up to three doses in the first hour 2, 1
  • For chronic symptoms, the European Respiratory Journal recommends 2.5 mg four times a day via nebulizer, with a maximum dose of 20 mg per 24 hours 3, 1

Considerations for Asthma Exacerbations

  • For mild to moderate exacerbations, an inhaler with a spacer is preferred over a nebulizer due to its cost-effectiveness and convenience, as stated by Thorax 4
  • For severe exacerbations, the dose can be increased up to 5 mg of salbutamol via nebulizer, according to the European Respiratory Journal 3, 5

Considerations for COPD

  • For mild COPD exacerbations, 200-400 μg of salbutamol (2-4 puffs) via inhaler with a spacer can be used, as recommended by the European Respiratory Journal 5
  • For more severe COPD exacerbations, nebulized salbutamol can be used at doses of 2.5-5 mg, as suggested by the European Respiratory Journal 5

General Considerations

  • The European Respiratory Journal recommends that for most indications of bronchodilator therapy, the use of a manual inhaler is preferred (with a spacer device if appropriate) 6, 4
  • Nebulization is generally reserved for patients who cannot use a manual inhaler correctly or who require high doses, as stated by the European Respiratory Journal 6, 3
  • Oxygen should be used as the driving gas for nebulization in cases of severe acute asthma whenever possible, according to Thorax 4

Salbutamol Dosage Guidelines

Dosage Recommendations

  • The British Thoracic Society recommends using a metered-dose inhaler (MDI) with a spacer instead of nebulization for routine use due to cost-effectiveness and convenience 7
  • The American Academy of Allergy, Asthma, and Immunology suggests that if salbutamol is needed more frequently than 2 days a week, it may indicate poorly controlled asthma and requires a reevaluation of the treatment plan 8
  • Increased need for salbutamol or decreased effectiveness may indicate worsening disease, according to the American Academy of Allergy, Asthma, and Immunology 8

Safe Usage Principles

  • Oxygen should be used as the driving gas when nebulizing salbutamol in acute severe asthma, as recommended by the British Thoracic Society 7

Pediatric Dosage

  • For children over 20 kg, the recommended dose is 5 mg per dose, as stated in the Thorax journal 7
