Esophageal Diseases Diagnosis and Treatment

Infectious Esophageal Diseases

  • The American College of Radiology recommends considering herpes simplex virus (HSV) esophagitis and cytomegalovirus (CMV) esophagitis in immunocompromised patients, presenting with shallow ulcers and deep ulcerations respectively 1, 2, 3
  • HIV-associated idiopathic ulcers are seen specifically in HIV-positive patients 1

Inflammatory Esophageal Diseases

  • Eosinophilic esophagitis (EoE) is characterized by eosinophilic infiltration, presenting with rings, exudates, furrows, and strictures, according to Clinical Gastroenterology and Hepatology 4
  • Lymphocytic esophagitis, hypereosinophilic syndromes (HES), and eosinophilic granulomatosis with polyangiitis (EGPA) are immune-mediated conditions that can affect the esophagus 5
  • Pemphigus vulgaris (PV) and lichen planus (LP) are dermatologic conditions that can present with esophageal involvement 6

Systemic Diseases with Esophageal Manifestations

  • Crohn's disease, scleroderma, systemic lupus erythematosus, mixed connective tissue disease, and Sjögren's disease are systemic conditions that can have esophageal involvement 5, 6

Diagnostic Considerations

  • Endoscopy with biopsy is the gold standard for most infectious and inflammatory conditions, as recommended by the American College of Radiology and MMWR Recommendations and Reports 1, 2
  • Biphasic esophagram and high-resolution esophageal manometry are essential diagnostic tools for detecting ulcers and motility disorders respectively 1, 6
  • Direct immunofluorescence (DIF) is diagnostic for dermatologic conditions like pemphigus vulgaris 6

Treatment Approaches

  • The American College of Radiology and MMWR Recommendations and Reports recommend treatment with fluconazole for fungal infections, and acyclovir or ganciclovir/foscarnet for viral infections 2
  • Clinical Gastroenterology and Hepatology recommends treatment with proton pump inhibitors, swallowed topical corticosteroids, or elimination diets for eosinophilic esophagitis, and condition-specific immunosuppressive therapy for dermatologic conditions 5, 6
  • Endoscopic therapy such as dilation, botulinum toxin injection, or myotomy is recommended for motility disorders 6
