Management of Progressive Kidney Disease in Hypertensive Nephropathy
Introduction to Treatment Strategies
- The European Society of Cardiology recommends intensifying treatment with the addition of an SGLT2 inhibitor, statin therapy for elevated LDL, and strict monitoring of kidney function and cardiovascular risk factors in patients with CKD stage 3b (eGFR 30 mL/min/1.73m²) and proteinuria 1, 2, 3
Blood Pressure Treatment and RAAS Blockade
- The European Society of Hypertension/European Society of Cardiology guidelines recommend a combination of perindopril 4mg, amlodipine 5mg, and metoprolol 50mg for hypertensive nephropathy 4
- Aim for blood pressure <130/80 mmHg in patients with proteinuria (0.34g/24u) 4, 5, 2
- Consider up-titrating perindopril to the maximum tolerated dose (up to 8mg) to further reduce proteinuria, provided potassium and creatinine remain stable 2, 3
SGLT2 Inhibitor Addition
- Empagliflozin, canagliflozin, or dapagliflozin are indicated in patients with eGFR 30-90 mL/min/1.73m² and proteinuria 1, 3
- SGLT2 inhibitors reduce the risk of kidney disease progression by 30% and cardiovascular death/heart failure hospitalization by 31% 1, 3
- The CREDENCE trial showed that canagliflozin reduced the primary endpoint (end-stage kidney disease, doubling of creatinine, or renal/CV death) by 30% in patients with eGFR 30-90 mL/min/1.73m² 1
Lipid-Lowering for Cardiovascular Risk
- Start a statin immediately in patients with CKD and high cardiovascular risk 3
- Aim for LDL <1.8 mmol/L (<70 mg/dL) in high-risk patients with kidney disease and proteinuria 3
- Consider a statin/ezetimibe combination if LDL target is not achieved with statin monotherapy 3
Proteinuria Management
- Aim for proteinuria <0.3g/24u through maximum RAAS blockade and sodium restriction 2, 3
- Restrict sodium intake to <2.0g/day (<90 mmol/day) to enhance the antiproteinuric effect of ACE inhibitors by 30-50% 3
Nephrological Referral
- eGFR <30 mL/min/1.73m² (stage G4) is an absolute indication for nephrological referral according to KDIGO 7, 6
- Progressive kidney disease requires specialist evaluation for possible kidney biopsies if etiology remains unclear 7, 6, 2
Monitoring Strategy
- Monitor creatinine, eGFR, potassium, and bicarbonate every 3 months 5, 3
- Monitor blood pressure at each visit, aiming for <130/80 mmHg 4, 5, 2
- Perform annual cardiovascular risk assessment, as CKD patients primarily die from cardiovascular causes 2
Nephrotoxic Agent Avoidance
- Avoid NSAIDs absolutely, as they accelerate kidney function decline 7
- Be cautious with contrast agents in patients with eGFR <30 mL/min/1.73m² 7
- Avoid oral phosphate-containing laxatives in patients with eGFR <60 mL/min/1.73m² 7
