Management of Elevated Baseline Intrauterine Pressure During TOLAC
Immediate Intervention
- Discontinuing oxytocin immediately is recommended when baseline intrauterine pressure reaches 40 mmHg during TOLAC with oxytocin, as per the American Academy of Family Physicians 1, 2, 3
- Repositioning the patient to left lateral decubitus position to optimize uteroplacental perfusion is advised 4
- Administering supplemental oxygen at 6-10 L/min is recommended 3
- Initiating intravenous fluid bolus if not already running is suggested 3
- Performing immediate vaginal examination to assess for rapid descent, cord prolapse, or signs of rupture is necessary 3
- Continuously monitoring fetal heart rate for Category II or III patterns is crucial 1, 2
- Considering tocolysis with terbutaline to reduce uterine tone if fetal heart rate abnormalities develop is an option 3
Decision Points for Continuing Labor
- If baseline pressure normalizes and fetal heart rate tracing remains Category I, cautious observation may continue, as per the American Academy of Family Physicians 1, 2
- If baseline remains elevated or Category II/III fetal heart rate patterns develop, proceeding to cesarean delivery is recommended 1, 2, 3
