Rifaximin in Colitis Treatment

Introduction to Rifaximin's Role

  • The British Society of Gastroenterology recommends against using rifaximin as primary therapy for ulcerative colitis or Crohn's disease, according to guidelines 1, 2
  • Rifaximin is approved as a second-line treatment for Irritable Bowel Syndrome with Diarrhea (IBS-D) in the USA, with modest efficacy for the FDA composite endpoint and stool consistency, but limited effect on abdominal pain 3

Clinical Applications of Rifaximin

  • The American Gastroenterological Association suggests against the use of rifaximin after acute uncomplicated diverticulitis, with a conditional recommendation and very low-quality evidence 6
  • Rifaximin is effective for non-invasive travelers' diarrhea caused by diarrheagenic E. coli, but less effective for invasive pathogens, and should not be used for dysentery regardless of illness severity 7

Safety and Efficacy Considerations

  • Rifaximin generally has a favorable safety profile, with adverse events similar to placebo in clinical trials, and headache as the most common adverse event 3, 4, 5
  • Retreatment with rifaximin is effective for patients with IBS-D who initially respond but later experience symptom recurrence 4, 5
