Treatment of Meconium Ileus in Newborns

Initial Management Approach

  • Parenteral nutrition may be essential as short-term support following intestinal resection in infants with meconium ileus, particularly when enteral feeding is not immediately possible, as recommended by Clinical Nutrition 1
  • Early enteral feeding should be encouraged to reduce risk of cholestasis, according to Clinical Nutrition 1

Post-Operative Nutritional Support

  • Pancreatic enzyme replacement therapy (PERT) must be initiated once enteral feeding begins, as over 90% of meconium ileus patients have pancreatic insufficiency, as stated by Clinical Nutrition and Praxis Medical Insights 1, 2
  • Fat-soluble vitamin supplementation (A, D, E, K) is necessary with pancreatic insufficiency, as recommended by Praxis Medical Insights 3

Cystic Fibrosis Confirmation and Long-Term Management

  • Sweat chloride testing (>60 mEq/L) confirms cystic fibrosis diagnosis, though infants often have initial values of 30-59 mEq/L, according to MMWR Recommendations and Reports and Praxis Medical Insights 3, 4
  • Meconium ileus occurs in 10-20% of newborns with cystic fibrosis and represents the earliest clinical manifestation (median age of diagnosis 0.2 months), as reported by Genetics in Medicine, MMWR Recommendations and Reports, and Praxis Medical Insights 2, 3, 5, 6
  • Immediate referral to a cystic fibrosis center for multidisciplinary care is essential once diagnosis is confirmed, as recommended by Praxis Medical Insights 3
  • Chronic airway clearance regimen and antimicrobial prophylaxis protocols must be established, according to Praxis Medical Insights 3

Meconium Ileus in Pediatrics

Diagnosis and Treatment

  • The American College of Obstetricians and Gynecologists notes that intestinal hyperechogenicity in fetuses can be associated with cystic fibrosis, although it also has other etiologies 7
  • The use of contrast enema with barium or Gastrografin can confirm the level of obstruction and show a microcolon, pellets of meconium in the terminal ileum 7

Treatment Outcomes

  • The American Academy of Pediatrics recommends that all patients with meconium ileus should be evaluated for cystic fibrosis before discharge, with a sweat chloride test (>60 mEq/L) confirming the diagnosis, although initial values in infants are often between 30-59 mEq/L 7
